Registration Procedure
1. Select the Al-Azhar Syifa Budi School branch by clicking the school logo above.
2. Fill out the Pre-Registration Form, then submit.
3. A confirmation email will be sent containing registration fee and payment instructions.
4. Make the registration payment via bank transfer and send the proof.
5. The school will contact to schedule the test.
6. Complete the online/academic test depending on the level.
7. Announcement of results.

TA–TK (Early Childhood & Kindergarten)
- Photocopy of Birth Certificate
- Photocopy of Family Card (KK)
- TK B applicants: TK A Report Card
- 2 color passport photos 3×4
Junior High School
- Currently enrolled in Grade 6 SD
- Birth Certificate & Family Card
- 2 photos size 3×4
- Report Cards Grades 1–5
- Transfer students: full report card, SD diploma, NISN
Senior High School
- Currently enrolled in Grade 9 SMP
- Birth Certificate & Family Card
- 2 photos size 3×4
- SMP report cards (Sem 1–4)
- SMP Diploma (Ijazah)
- NISN Card
- Study program recommendation letter
